Description: We keep getting the occasional feedback message where the person submitting it seems to expect a reaction / an answer, but fails to supply information on how we can reach them (latest example: PF-2061).

While it should be obvious to anybody making a request that this will not be possible without contact information, maybe we can make this a bit easier for them by, e.g.
- amending the text in the grey box from "Please provide your feedback below:" to something like "Please provide your feedback below. If you would like a response, please do not forget to also supply an email address."
- pulling the two form items for name and email above the description block. In my browser, e.g., it is not too obvious that they even exist, only the scroll bar suggests that there could be more below. By default, they are not on the visible part of my form, and I can click "submit" without ever coming by them.
