I need help locating this form. Was it in the the occurrence search page? Currently this page doesn't seem to work (it shows this message: "Page Not Found. We are sorry, but the page you requested does not exist.")

Ok, I added the date dropdown to the months. If you want to add it to the years too, add the class .date-dropdown to the select box:
